PRN RN Care Manager
We are currently seeking a PRN RN Care Manager for Jennings Home Care. The Care Managers will be responsible for providing skilled nursing care to assigned patients including assessment and development of the plan of care, evaluation, patient/caregiver education and discharge planning in accordance with the agency policies and procedures and the physician care plan. Performs admissions and assumes responsibility for managing a caseload of patient; maintains timely communications with others involved in the care of the patient, including updates and revisions to plan of care to facilitate achievement of expected outcomes. Provides supervision to LPNs, home care aides and family/caregivers involved in the care of the patient. Participates as a member of a patient -focused interdisciplinary care team. Position includes some on-call rotation, including weekends and holidays. Future potential opportunities exist for this to become full time employment. Qualified candidates will hold a current Registered Nurse (RN) license in good standing in the State of Ohio; and a minimum of 1 or more years experience in an acute MED/SURG setting. Previous Home Care experience is strongly preferred.
