Trinity Community of Beavercreek is seeking a MDS RN for our faith based, nonprofit organization owned by United Church Homes, Inc. At Trinity Community our mission is to transform Aging by building a culture of community, wholeness and peace. We are looking for an experienced MDS Coordinator to help us in our mission. We are conveniently located just 5 minutes east of The Greene shopping center on Indian Ripple Road.
Key Responsibilities:
- Coordinate care deliver interventions for residents in their assigned area.
- Ensure resident care plans are in accordance with current professional practice standards, physicians orders, facility policies and procedures along with local, state, and federal regulations.
Key Qualifications:
- At least six (6) MONTHS experience in long-term care and MDS experience is required.
- Current, unencumbered, active RN license in Ohio.
- Must be licensed in accordance with state & federal laws.
- Must meet pre-employment requirements of United Church Homes, Inc. and all other federal and state requirements.
