MDS Nurse 

Summary

The MDS Nurse coordinates the development, completion and transmission of assessments in accordance with the regulatory requirements and reviews and manages reimbursement.  Responsible to case manage care plans, including rehabilitative programs.  Must demonstrate knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare participation.  Must be comfortable to function as an effective team member through good communication and cooperation.  Sufficient computer skills required for performing electronic medical record/submission duties. 

Responsibilities:                  

  • Fully understands the philosophy of the small house
  • Effectively complete the MDS and Care Plans
  • Track Physician and Ancillary Visits and Orders
  • Discusses elder observations/concerns with appropriate coordinator.
  • Attends nurse team meetings
  • Generate Monthly MDS Calendar and Notification Process for Support Staff
  • Provides coaching and direction to staff as needed.
  • Discharge planning for elders including home services upon discharge
  • Completes the certifications and recertification’s for elders in the neighborhood
  • Facilitates the weekly Medicare/elder meetings
  • Facilitates the weekly Medicare/therapy/Neighborhood meetings
  • Acts as the therapy liaison
  • Completes the reassessments for elders and hospital visits when appropriate
  • Provides training materials to staff and team when appropriate
  • Collaborates in the development of new processes, forms, and systems
  • Collaboratively identifies issues and develops actions plans for groups and individuals
  • Facilitates the restorative program at the neighborhood
  • Participates in the morning stand up meetings
  • Assists in the management of the survey process including 672 and 802 reports
  • Performs any other duties as assigned.

  • Make prompt and accurate judgments with regard to elder care and emergencies.
  • Work and communicate effectively as part of the self-directed work team.

Qualifications

  • Valid Ohio RN
  • CPR certification
  • 2 years’ recent experience as an MDS nurse required.
