Shared Concepts to Effectively Communicate Frailty for Hospice and Palliative Care Provider

The clinical concept of “frailty” is an important factor in the care and management of all Medicare beneficiaries. This session will define the concept of frailty, discuss how the concept of frailty applies to Medicare hospice claims and describe the steps necessary to improve the communication of frailty using a provider-payer collaborative approach.

Your registration will be complete upon receipt of registration and payment. All registrants will receive information for the virtual session upon registration. If you do not receive a confirmation, please contact Corey Markham at (614) 545-9015 or cmarkham@leadingageohio.org.

Substitution and Cancellation Policy: Substitutions, in writing, are welcome at no additional cost.  Cancellations received by two calendar weeks prior to event date will be fully refunded. Cancellations received between 7 and 13 days prior to the event date will be refunded minus a $40 processing fee. No refunds will be issued 6 or fewer days prior to the event date.

Continuing Education Information

1.0 contact hour will be awarded upon successful completion.

1.0 clock hour will be awarded upon successful completion. Pending BELTSS approval.

Criteria for completion: Attendance at entire event and submission of completed evaluation form.

Other states may deem the Ohio credit acceptable, but participant must check with his/her local licensing board.

No conflict of interest exists for any individual in a position to control the content of the educational activity.

LeadingAge Ohio is an approved provider by the Board of Executives of Long Term Services and Supports. Core of knowledge subject areas: Resident Centered Care and Quality of Life; Leadership and Management.

As stated in the Ohio Administrative Code Rule 4723-14-05, education that has been approved by a board or an agency that regulates a health care profession in Ohio or another jurisdiction is an acceptable option to meet nursing continuing education requirements.

LeadingAge Ohio is approved as a provider of nursing continuing professional development by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. (OBN-001-91)
