A recent Joint Commission Deemed Status Hospice Survey resulted in condition-level citations related to deficiencies in plans of care and interdisciplinary group (IDG) meetings.  The team, with every discipline represented, completely revamped their care planning and IDG meeting process.  This monumental change resulted in plans of care with individualized, patient centered, and measurable goals and IDG meetings with efficient yet effective summary templates that impressed future surveyors.  They also adjusted their assessment process and documentation to capture symptom severity on every visit, directly reflecting the status and progress towards the goals in their care plans.   Learn how to develop your own step-by-step process to achieve compliance with regards to plans of care, IDG meetings, and assessments.

Substitution and Cancellation Policy: Substitutions, in writing, are welcome at no additional cost. Cancellations received by two calendar weeks prior to event date will be fully refunded. Cancellations received between 7 and 13 days prior to the event date will be refunded minus a $40 processing fee. No refunds will be issued 6 or fewer days prior to the event date.

Continuing Education Information

2.0 contact/clock hours will be awarded upon successful completion.

Criteria for successful completion: Attendance at entire event and submission of completed evaluation form.

Credit awarded commensurate with participation, as allowed by professional board.

Prorated contact hours will be considered in special circumstances, as allowed by professional board.

No conflict of interest exists for any individual in a position to control the content of the educational activity.
