Second Quarter 2019 Immediate Jeopardy Citations

Data on immediate jeopardy (IJ) citations issued by the Ohio Department of Health (ODH) from the second quarter of 2019 are now available. Immediate Jeopardy refers to those situations in which a provider’s noncompliance with one or more requirements of participation has caused or is likely to cause serious injury, harm, impairment or death to a resident.

In total, 22 Ohio facilities received 32 IJ citations. Two facilities had more than one survey with IJ citations in the second quarter: one facility received citations in F689 and K925; another facility received citations in F600, F607, F609, F610, and F835 with the second survey receiving citations in F623, F624, and F835.  There were two additional facilities that received more than one IJ citation during the survey: one received the IJ citations in F583 and F600; the other received the IJ citations in F600 and F624. The scope & severity citations included twenty-three (23) J’s and nine (9) L’s.

LeadingAge Ohio has prepared a summary of findings which includes key areas cited and contributing factors surrounding the citations. The scope and severity levels are also included within the summary.
