Trinity Community of Beavercreek is seeking a MDS RN for our faith based, nonprofit organization owned by United Church Homes, Inc. At Trinity Community our mission is to transform Aging by building a culture of community, wholeness and peace. We are looking for an experienced MDS Coordinator to help us in our mission. We are conveniently located just 5 minutes east of The Greene shopping center on Indian Ripple Road.

Key Responsibilities:

Coordinate care deliver interventions for residents in their assigned area.
Ensure resident care plans are in accordance with current professional practice standards, physicians orders, facility policies and procedures along with local, state, and federal regulations.

Key Qualifications:

At least six (6) MONTHS experience in long-term care and MDS experience is required.
Current, unencumbered, active RN license in Ohio.
Must be licensed in accordance with state & federal laws.
Must meet pre-employment requirements of United Church Homes, Inc. and all other federal and state requirements.
