Piecing it Together:
Payment Innovation in Affordable Housing

Join us for a day of mapping solutions for funding community-based aging services. Ohio’s aging population will reach its peak by 2035, while its working-age population contracts. Communities will need to optimize existing, low-cost care settings with just-enough services that maximize older adults’ wellness and function, staving off reliance on health systems. The day’s agenda focuses on innovations that housing operators have already deployed to secure payment for the services they provided to residents. The day will begin with a high-level view of where aging services is headed and what new practices offer the most promise. On-site service coordination is one solution, and participants will learn how different partnerships have worked—or not worked—to create sustainable funding streams for this vital service. Later in the day, participants will hear about different models that allow healthcare providers to assume full risk for the population they serve—and the cost-effective savings. Don't miss this opportunity to be at the table.

The day will end with participants rolling up their sleeves to build on what they've learned and chart their next steps towards a better system of care. Small groups will fit the pieces together creating a clear vision for the future of community-based care.

Additional Workshop Information

5.0 contact hours will be awarded upon successful completion.

Criteria for successful completion: Attendance at entire event.

As stated in the Ohio Administrative Code Rule 4723-14-05, education that has been approved by a board or an agency that regulates a health care profession in Ohio or another jurisdiction is an acceptable option to meet nursing continuing education requirements.

Substitution and Cancellation Policy: Substitutions, in writing, are welcome at no additional cost. Cancellations received by two calendar weeks prior to event date will be fully refunded.
